Plans for the 2006 Canadian Charity Ball are underway and the event is scheduled to take place on Saturday, May 20 2006.

The Canadian Charity Ball Committee is currently accepting funding proposals from non-profit organizations in need of financial support.

The Canadian Charity Ball is an annual charity event established to support non-profit organizations operating in China. In previous years, the generosity of Canadian Charity Ball participants and sponsors have funded various development projects throughout China.

Projects are determined through a process of review and selection by the Canadian Charity Ball Fund Committee. Proposals are selected based on the on their:
1) financial need;
2) organizational capacity to successfully implement the project(s);
3) potential to make a significant impact and contribution to individuals or communities in need of support.

The scope of past projects have included the support of education, health and provision of basic needs to individuals and/or communities in need of support.

This year, the Canadian Charity Ball is looking to support:

One major charity funding project of up to 600,000 RMB; and/or*
A combination of small (under 100,000 RMB) and medium (100,000 to 250,000 RMB) size charity projects.
(* Project(s) selected to be determined by the quality of applications submitted and the fundraising targets reached after the event.)

A) GENERAL INFORMATION ON NON-PROFIT ORGANIZATION
1) Name of Organization:
2) Contact Details: Contact person, address, phone number, fax number, e-mail and website address if applicable.
3) Brief Introduction of the Organization: (300 words or less) e.g. Date of establishment, mission and objectives of the organization, charity work done, proposed work to be done, overall impact of charity work in the community.
4) Size and Structure of Organization: Please describe your organization's size and structure. Provide details as to how many staff, members, and volunteers work in your organization? Submit an organization chart if available.
5) Sources of Funding: Please describe your sources of funding. Do you receive funding from government, business or public sources? Does your organization engage in fundraising activities?

B) FUNDING PROPOSAL
1) Amount of Funding Requested: Please provide figure in RMB.
2) Proposal Letter (in 3000 words or less): Please provide a detailed letter explaining why your organization should be selected as a supported charity of the Canadian Charity Ball. Detail why your organization is in need of support. e.g. Why is your work important compare to other charity causes? Explain how your project(s) will make a significant impact and contribution to individuals or communities in need of support. e.g. Please describe the impact your project will have in the community. Who will benefit most from the project?
3) Project Budget and Summary: Please provide a detailed budget of your project. Please describe how you will use the funds. Why is the requested amount needed? Please list components of the project and outline which elements and aspects the funds will go towards.
4) Project Timeline and Deliverables: Please provide a project timeline and describe tangible products or results your organization will seeks to accomplish with the funds.
